HealthTeam Advantage member identification cards - you will receive your HealthTeam Advantage member identification card in the mail.
A Personal Healthcare Concierge at your service - if you would like assistance finding an in-network provider, scheduling an appointment, have questions about your benefits, or need a replacement identification card, simply email your concierge at email@example.com, or call your Healthcare Concierge at 1-888-965-1965 (TTY 711). Our hours of operation are:
October 1 - March 31: 8am to 8pm, EST, seven days a week
April 1 - September 30: 8am to 8pm, EST, Monday through Friday.
